This entity has many different faces, which coalesce in the lung and become difficult to distinguish.

Whether preexisting pulmonary disease, (like Tbc, cystic fibrosis, bronchiectasis…), or simply abating immune response of advancing age, they all predispose to this infection.

Central newly appearing bronchiectasis of big caliber, and peripheral lung infiltrates (Image opposite to lung edema) are typical landmarks.

Peripheral infiltrates are also typical of Covid pneumonitis, but involve mostly lower areas.

This especially in a patient with wheezing without previous asthma, and eosinophilia in blood smear should prompt investigation in this direction.

A CT scan belongs to work-up.
